Bad customer service

"I ordered about $100 worth of items in Nov. for Christmas gifts. Upon ordering I was told 1 of the items was on back order( a $19 T-shirt) that it would ship separately when it came in and would be billed separately. All the items except the T-shirt arrived in about 10 days & I was satisfied with it all. I noticed a few days later that the T-shirt had been charged to my Visa; when it didn't arrive after 3 weeks I called and asked, was told it had shipped and been delivered. When I explained that I had not received it, I was told they would mail me a form I had to fill out and sign and then I could have my card credited or they would send another T-shirt. Either was fine with me. 10 days pass no form. I called, Amy the supervisor said ''oh we'll send another" I had them verify my address, they had it in their system correctly. Still no form. I give up. Too bad too. I liked the items that I did receive.
