php issue i dont understand (phpbb)

When people try to register on my forum, they are taken to a form they have to fill out where they can choose username, email, password etc..

For some reason the 'username' box is not showing up, so people cannot enter a username and when they click 'submit' they are told that the username has not been entered.

I only recently noticed this problem, so I am not sure which modification caused this problem.

But if anyone is familiar with php and phpbb I would really appreciate some help.
