Systems D3D Initialization error

Everytime I try to load Dungeon Siege, I get a "D3D" Initialization error.

Motherboard: ASUS 7600 ( I Think)]
Graphics Card: Hercules 4000XT 32 MB PCI
OS: Windows XP
DirectX 9

I have run dxdiag. Everything is set to "Enabled"
I have run the D3D test and it gives me the following message:

"Failure at step 8 (creating 3d Device) HRESULT=0x80004005 (Generic Failure)"

Would upgrading to a better, AGP slot graphics card help?

I have tried upgrading to service pack 1 but that only causes my computer to crash so I had to uninstall the service pack in safe mode.

I would try upgradeing the drivers for that card. These should be correct..

See if that helps your problem, if not, it may be a compatibility issue with that video card and replaceing the card may be necessarry.

also, why did service pack 1 crash? what error?
