In times past, I have had this happen. I was never able to track it down and prove, but seems to be the modem I was using at the time (PCI Winmodem with Rockwell chipset). I could go off line and then reconnect but that usually didn't help either, a reboot would get everything going again...for awhile. That modem (dialup) was flaky in other ways, so I eventually bought a good USR hardware modem, and have never looked back...so..if I had to make a WAG, I would say your problem may be with the modem you use.
