Fan controller and fan failure

Could a fan controller burn out fans? I installed a SystemMaxx fan controller a couple of months ago and noticed a few days ago that the LEDs would blink and fans would restart. I thought it was just an ancient 120mm fan failing and ignored it. Opened up today to discover 3 of the 4 connected fans are not spinning.

I have a POWMAX Ultra 500 PSU so that should cover the load. Is the controller Failing?

O h crap the fourth fan failed as I was writing and the LEDs are totally dim now.

I guess I will have to rip it out and test the fans. Anyone have similar experience?

Almost sounds like an over-voltage problem.

Ok so when I removed two fans and connected them to the MOBO al four fans spin fine. So can I do to get this controller working?
