There are a couple troubleshooting things I would try.
First step is to remove the modem through windows. Power off the machine, open your case, switch the ISA or PCI slot the modem is using, power computer back on , let it detect/install the modem on its own if possible, and then you want to perform a diagnostic on the modem (start/settings/control panel/modem) Then go to diagnostics/ query modem. See if the modem talks back to you.
Then I would attempt to dial out again and see if the problem still continues.
One more thing I would do to troubleshoot is change the power supply. I was having issues with my PC restarting forever for no reason, the minute I changed my power supply..the problem was gone.
