There was a duplicate record in the table.... I removed the duplicate so you should be able to go back through the survey and finish it if you want.
This is the first I have seen of this problem... but I'll keep an eye out for any reoccurrences, and find what is causing the problem if so. My guess you didn't cause this and won't ever see this error again.
If you go back to review a company you have reviewed before, it displays your previous answers and if you change anything, it updates the original answers you made.
