Have similar problems with lsass.exe even though my problem is intermittent and it came up after updating video drivers for my Nvidia Geforce3 card.
Now my computer sometimes stop the booting process just before the Welcome screen with the lsas.exe popup and something about “operation failed”.
Have tried all kinds of things, including turning of all kinds of services, but that didn’t help.
I can get the computer to consistently reboot correctly if I completely uninstall the drivers for the video card, (and ignore the “found new hardware balloon”)
I’ve seen other posts about lsass.exe problems after updating video drivers.
Strange also that there’s no mention of these problems in the Xp event viewer. The bootlog (which I turned on) does differ in the way that when the boot fails you can see that it does not load the afd.sys driver.

Can anyone see the connection between these files, drivers, hardware etc?
Annoying running 640x400 screen res. after a while….
